Unexplained infertility

Understand a diagnosis made when standard fertility assessment finds no evident cause.

Editorial publication · Medical review pending.

Understand

Unexplained infertility means no evident cause has been found after an appropriate basic fertility assessment. It does not mean no mechanism exists or that anyone is at fault.

Possible symptoms

The only sign may be not achieving pregnancy. By definition, standard assessment of ovulation, tubal patency and semen does not explain the difficulty.

Causes and associated factors

Subtle factors involving gametes, fertilisation, embryos or implantation may escape routine tests, but unproven hypotheses should not be presented as diagnoses.

Diagnosis

It is a diagnosis of exclusion after reviewing the history, ovulation, tubes and semen, with assessment tailored to age and duration of infertility.

Tests

The basic work-up should be complete before using this label. Many additional immune, genetic, microbiological or implantation tests are not recommended routinely because evidence is insufficient.

Course of the condition

Spontaneous pregnancy remains possible. Prognosis varies with age, duration of infertility and previous pregnancy; these factors guide decisions between waiting and treatment.

Care options

Depending on the situation, individualised expectant management, stimulation with insemination or IVF may be discussed. Automatically adding unproven treatments can increase costs and risks without demonstrated benefit.

When to consult

Ask which tests were actually completed, why the diagnosis applies and how your untreated prognosis was estimated before choosing an intervention.

Urgent help

This diagnosis is not an emergency. During treatment, significant abdominal pain, rapid swelling, breathing difficulty, heavy bleeding or collapse needs urgent advice.

Questions for your doctor

  • Which cause or combination of factors explains our fertility difficulty?
  • Which tests are genuinely useful before deciding?
  • Which options exist before or instead of IVF?
  • Which benefits, limits, risks and costs should we compare?
  • What support and reassessment timeline should we plan?
