Understand

An ovarian cyst is a sac, usually containing fluid, on or within an ovary. Most are benign, but their nature needs assessment.

Possible symptoms

It may cause no symptoms, or pelvic pressure, pain, bloating or urinary discomfort. These signs alone cannot identify a cyst.

Causes and associated factors

Functional cysts are linked to the menstrual cycle. Others arise from different ovarian tissues or are associated with endometriosis. The mechanism and course depend on the cyst type.

Diagnosis

Assessment considers symptoms, medicines and the last menstrual period. Examination and pelvic ultrasound describe the cyst’s location, size and appearance and look for other explanations.

Tests

Depending on ultrasound findings, blood tests or MRI may be offered. Blood markers alone cannot diagnose cancer. The tests needed vary with the situation.

Course of the condition

A functional cyst often resolves spontaneously; a check may assess its progress. After menopause, criteria differ: some small simple cysts need no regular monitoring after assessment.

Care options

The decision depends on symptoms, the cyst’s appearance and size, and menopause status. Monitoring or surgery may be discussed. Before an operation, discuss whether the ovary can be preserved and the possible effects on fertility.

When to consult

Seek advice for persistent pain, pressure or other symptoms. Mention unusual bleeding and attend recommended checks even if you feel better.

Urgent help

Severe or worsening pelvic pain, fainting, heavy bleeding or breathing difficulty need immediate help. With vomiting, fever or possible pregnancy, seek urgent advice. Do not wait for the planned cyst check.

  • Do I need a check and when?
  • Why recommend surgery or monitoring?
  • Can the ovary be preserved?
  • What are the effects on fertility and the follow-up plan?
